Access the Current Survey
Please keep in mind that the online survey must be completed in one session. If you leave the survey at any time, you will have to start it from the beginning.
To help you prepare for the survey, we are providing you a link to a PDF version of the questionnaire.
We strongly recommend that you print and review the questionnaire before clicking on the survey link, but do not fill out the paper version and mail it in, as it will not be accepted. Click on the Print button to the left to print and review the questionnaire.
After having reviewed the questionnaire, please click on the link below to complete the online survey. Surveys submitted only through the web site will be accepted.
Once you have completed the survey, you will have the option to either email your survey responses to your email address or to have them printed (for this latter, you will need to have your computer connected to a functional printer). Due to the length of the survey, we suggest that you email your survey responses. This is the only time that you will be able to utilize the email/print report function; you will not be able to retrieve your report later from the survey web site. Please keep in mind that the research team at no time will have access to your individual report and will be unable to provide you with a copy.
